About the Provider
David Tomazic, DO, spent nearly 30 years in a solo private practice in Forest City, PA prior to joining Wayne Memorial Community Health Centers in 2017. In addition to outpatient primary care, he serves as the Medical Director of the Forest City Nursing and Rehabilitation Center. Dr. Tomazic also held the position of Medical Director of the State Correctional Institute in Waymart for 13 years.
