About the Provider
As a certified family nurse practitioner, Sara Kovalchik, DNP, MSN, CRNP, FNP-C, is an advanced practice provider with educational and clinical training in the specialty of pediatrics. Sara sees patients from birth to age eighteen years old. She provides diagnosis, treatment and management of acute and chronic illnesses as well as monitoring of childhood growth and development, performing routine physicals and administering immunizations.
Sarah joined Wayne Memorial Community Health Centers in 2025.
