About the Provider
As a certified physician assistant, Taylor Hook, PA-C, is an advanced practice provider working under a physician’s supervision. Taylor provides medical care to children from birth to young adulthood. She performs physical exams, diagnoses and treats acute and chronic illnesses, orders and interprets tests as well as administers treatments and immunizations.
Taylor joined Wayne Memorial Community Health Centers in 2025.
